Westmoreland County, the United Way, your local EMS providers, and Excela Health are pleased to provide COVID-19 vaccinations to individuals who are homebound. We are able to immunize those individuals who are unable to travel in their homes but we need your assistance. We know that you are in the best position to know the needs of your clients and have means to get in touch with them. We are asking for your help to contact these individuals to ascertain their willingness to accept the vaccination. For those who are willing, we will need the following information:
This information will be shared with the county in order to devise vaccination schedules that will optimize utilization of the vaccination. Once we determine scheduling, we will notify you so that you may notify your clients. Individuals will be receiving the Moderna vaccine which is appropriate for those 18 years of age and older. Moderna vaccinations require a second dose 28 days later that will be scheduled at the time the initial appointment is being made. We do ask that patients be free of COVID-like symptoms (fever, chills, cough, body aches, new headache, loss of taste/smell) at the time of vaccination AND that they not be in quarantine for COVID 19 exposure (quarantine typically lasts 10-14 days since last exposure) OR in COVID 19 isolation (in general, individuals diagnosed with COVID 19 must remain in isolation until it has been at least 10 days since symptom onset, their symptoms are improving, and they have been fever free for at least 24 hours without the use of fever reducing medications. They must be willing to allow a vaccinator to enter their home. We would ask that individuals make every effort to restrain pets during vaccination.
Please provide your list of homebound individuals by April 21, 2021, to:

Once we receive the lists we will be able to create our vaccination schedule and get shots into the arms of our neighbors who are the most vulnerable!
On behalf of the County, the United Way, your EMS providers, and Excela Health, thank you so much for working with us to improve the health of every life we touch!
