Long Term Disability
When There is a Claim
When a claim is to be made, Human Resources, rather than the insurance company, should be notified without delay.
Termination of Coverage
Your coverage will terminate if you cease to be an eligible employee or if the Benefit Plan is discontinued.
If you should die, the Long Term Disability Coverage of the Group Policy may be continued for your covered spouse or any unmarried children less than 25 years old for three months, subject to the same conditions that would apply if you were living.
Long Term Disability Coverage
Your weekly benefit under the self-funded program is 60% of your weekly earnings, rounded to the next higher $1. Benefits will be paid on a monthly basis. Your monthly benefit under the group policy is 60% of your monthly earnings.

Should you be entitled to periodic benefits from certain other sources during a disability, your monthly benefit may be reduced, as explained later.
Under current law, your monthly Long Term Disability Coverage is subject to social security tax during the first six months of disability. You may also request that federal income tax be withheld from the amount otherwise payable. The monthly benefit shown above will be reduced by the amount withheld. Should you be entitled to periodic benefits from certain other sources during a disability, your monthly benefit may be reduced, as explained later.
Elimination Period
The elimination period is 90 calendar days following the exhaustion of your earned sick, personal and vacation days.
Under the Group Policy issued by Mutual of Omaha, benefits will begin after the elimination period and approval by HR.
